American Care Premier Plans Program Cost

All contracts have a one-time, non-refundable processing fee of $30 (due at time of contracting).

The cost is $30 per month per individual. (Due the 1st of each month.)

[Includes one electrocardiogram, one CBC, one chemistry panel (kidney function, liver function, glucose/sugar), one cholesterol level, and one urinalysis.]
Transportation to the network Medical Center visit	$7
Physical Therapy office based	$15 per treatment
